Teyneen Taylor was a mental health campaigner and raised awareness of mental health by blogging about her experience with borderline personality disorder. She died in October 2018 at a psychiatric hospital after a second incident of self-harm. At the inquest, the jury found her death was accidental but that her care was insufficient. Given the high risk of self-harm, removal of items from her room and monitoring were insufficient.
