Emma-Louise Fenelon was instructed by the family of Regan Tierney, who was murdered by her former partner in 2019. After 7 pre-inquest review hearings, an inquest was held in July 2024 at Bolton Coroner’s Court before Area Coroner Peter Sigee.

The jury returned a conclusion of unlawful killing, criticising Greater Manchester Police for inadequate information sharing, insufficient professional curiosity, inaccurate DASH risk assessment and unacceptable information recording.

The jury found that Ms Tierney should have been offered a possible Non-Molestation Order or Restraining Order, a referral to an Independent Domestic Violence Adviser and a Multi-Agency Risk Assessment Conference, interventions that would possibly have prevented her death.
